Giving to the Hope Fund

It’s easy to make a difference

When you’re giving a gift from the heart, the last thing you want to worry about is deadlines and details. That’s why we’ve structured the HCA Hope Fund to be as flexible and easy as possible. Whether you wish to make a one-time gift with stock, check or credit card, or have your gift deducted from your paycheck a little at a time, your contribution will make an important difference. Every bit helps.

It's as easy as 1-2-3:

1) Decide how you want to give.

2) Download the donation form below.

3) Complete the form and return it to the address stated on the form.

There are several ways to give:

1) Click here to make a credit card donation online.

2) Donate to the fund with a personal check using a paper form.

3) Spread out your contributions over time by using payroll deduction. 

4) To make a stock donation, please contact the HCA Hope Fund staff at (615) 344-1274 for details.

5) To make a change or stop your ongoing payroll deduction complete the Change/Stop form and submit to your HR department.

Giving at a glance

  • You can contribute to the Hope Fund at any time.
  • All donations are tax-deductible.
  • There's no minimum amount you must meet to give. Every bit helps!
  • Payroll deduction pledges begin with each New Year and continues until you request a change or stop to your deduction. For now, you may pledge your deduction, per pay period, on the payroll deduction form. Complete this form and return it to your HR department.
  • To change or stop your deduction please complete the Change/Stop form and return it to your HR department.
